Uniting AgeWell Melbourne West Metro Home Care is a home care service supporting older people in Melbourne's west to remain living safely and independently at home. It covers the regions of Brimbank, Hobsons Bay, Maribyrnong, Melton, Moonee Valley, Port Phillip, and Wyndham. Services include the Support at Home program, Commonwealth Home Support Program (CHSP), respite and carer support, care management and navigation, allied health, and social support. As an organisation of the Uniting Church in Australia, Uniting AgeWell has been delivering aged care services for more than 70 years across Victoria and Tasmania. The service was voted Most Trusted Brand in Home Care Vic/Tas in the 2025 Reader's Digest Trusted Brands Survey.
