HammondCare At Home Western Sydney

HammondCare At Home Western Sydney is a home care service delivered by HammondCare, an independent Christian charity that has been providing care for over 90 years. The service supports older people to remain in their own homes by offering nursing and clinical care, dementia support, respite care, palliative care, allied health services (including physiotherapy and dietetics), and practical home help. Funding is available through the Commonwealth Home Support Programme, the Support at Home program, or on a private basis. HammondCare also provides pastoral care for people of all faiths and runs Social Clubs offering activities and peer connection while carers take a break.
