Aged Care At-Home Link

Aged Care At-Home Link is a free, independent aged care navigation and referral service based at Suite 3, 475 Ruthven Street, Toowoomba, QLD 4350. It is not a residential aged care home; instead it helps older Australians and their families navigate the My Aged Care system and apply for government funding such as the Home Care Package (HCP) and Commonwealth Home Support Programme (CHSP). The service guides clients through ACAT/ACAS assessments, eligibility and income-assessment steps, and then connects them with trusted home care or residential care providers. All support is offered at no cost to the client.
