HammondCare Northern Beaches Palliative Service

HammondCare Northern Beaches Palliative Service is a specialised community palliative care service operating from the Cora Adcock Centre on the Mona Vale Hospital campus in Sydney's Northern Beaches. The service provides multidisciplinary care to palliative patients in their own homes across the Pittwater, Manly and Warringah shires, including pain and symptom management, coordination of hospital or hospice admissions, equipment provision, social support, counselling, and volunteer support. A dedicated Palliative Care Clinic offers additional social, spiritual, emotional and physical support to registered patients and their carers. Referrals are accepted only from GPs, medical specialists, or advanced practice nurses, and the service operates Monday to Friday, 9am to 5pm.
