Uniting AgeWell Grampians Home Care

Uniting AgeWell Grampians Home Care is a home care service based in Ballarat Central, Victoria, supporting older people across the Ballarat, Pyrenees and Ararat local government areas to remain living safely and independently at home. It offers Support at Home and Commonwealth Home Support Program (CHSP) services, including care management, rehabilitation and allied health support, all arranged through a tailored care plan developed with a Uniting AgeWell Care Advisor. The service also runs community transport for clients unable to use public transport, covering trips to medical appointments, the hairdresser, shops and community groups. Uniting AgeWell is a not-for-profit organisation of the Uniting Church in Australia and was voted Most Trusted Brand for Home Care in Vic/Tas in the 2025 Reader's Digest Trusted Brands Survey.
