4 Island Aged Care

4 Island Aged Care is a home care agency established in 2022, operating under a Community Self-Managed model to deliver Home Care Package services to clients in the Southern Moreton Bay Islands (SMBI) and Bribie Island areas of Queensland. The service prioritises using local workers, giving clients the flexibility to choose who provides their care, including people they already know. A local Community Support Coordinator is available to assist clients with appointments, sourcing aids or equipment, and resolving care-related issues. The model is designed to strengthen the local community through collaborative support arrangements. No residential aged care facilities or on-site accommodation are offered.
