HammondCare Caulfield is a specialised residential aged care facility located at 294 Kooyong Road in Caulfield, Melbourne, offering expert care with a strong focus on dementia and palliative care. The facility includes Caulfield Village, described as one of the only purpose-built dementia care villages in Melbourne, featuring small home-like cottages surrounded by gardens and designed with evidence-based principles to support independence and a sense of familiarity. The Glens is a separate double-storey care home on site, providing care across five living zones of up to 15 people each for those with complex health needs.
