Holistic Disability Solutions is an NDIS-registered, nurse-led disability and aged care support provider operating across Port Macquarie, Wauchope, Kempsey, Taree, Forster, and the wider Mid North Coast of NSW. They deliver in-home supports including daily personal activities, high-intensity personal care, community nursing, continence assessments, and medication management under Registered Nurse oversight. The service specialises in participants with complex and high-intensity needs, including behaviours of concern, complex physical health conditions, and situations requiring close clinical monitoring. They also support community participation, employment pathways, and education goals in a person-centred, consistent manner. This is a community-based support service, not a residential aged care facility.
