Juniper Transition Care Karrinyup

Juniper Transition Care Program provides short-term care for up to 12 weeks to help people recover and return home after hospital discharge. Available in Perth and Albany, the program offers residential accommodation at locations like Juniper Karrinyup (with private ensuites and meals included) or in-home support, or a combination of both. Multi-disciplinary teams provide nursing, physiotherapy, occupational therapy, personal care, and domestic assistance tailored to individual rehabilitation goals. The program aims to improve health and mobility so participants can continue living independently. Basic daily fees are $13.08 for in-home services or $63.82 for residential accommodation.
